The Ministry of Health proposes to give Roszdravnadzor the appropriate powers in relation to state medical institutions, but in fact, according to experts interviewed by MN, it is trying to shift the responsibility for failed healthcare reform to doctors.
The size of fines will be from 10 to 20 thousand rubles. According to the data provided in the documents of the Ministry of Health, in just one year, more than 47 thousand complaints were received regarding the quality and accessibility of medical care. On the whole, the initiative suggests that now, if Roszdravnadzor reveals a discrepancy between treatment practice and established standards, it will have the right to fine a medical institution. The same fine may be imposed on the official responsible for the violation.
However, as Sergei Leonov, a member of the Federation Council Committee on Social Policy, emphasized at a press conference in Moscow, this document, which has not yet been discussed by the upper house of the Federal Assembly, raises many questions.
In particular, the senator emphasized, it seems (and other experts adhere to this opinion) that in fact the government is trying to remove the Ministry of Health from the area of responsibility for certain flaws in the provision of medical care, transferring it to doctors of medical institutions and indirectly to regional authorities of Russian regions Federations that oversee health issues.
– Fines per se, maybe small, from 10 to 20 thousand for each case, but if the hospital is sufficiently visited and the amount of such fines for the same head doctor is at least a dozen per month, then, you know, for any head doctor already large – from 100 to 200 thousand – money. Therefore, I consider this measure redundant, it should be discussed. But the main thing that is clear right now: the initiative of the Ministry of Health does not correct the situation, especially in the primary care, ”the Russian senator said.
Another expert, President of the Doctors Defense League, Semyon Halperin, drew attention to the fact that there were precedents in practice when fines were imposed on the courts in a hospital, and then the hospital collects this money from doctors:
– I think that with the implementation of this initiative, any situation can be expected, because it is not known what the law enforcement practice will be and what we really get. But in any case, the idea of the Ministry of Health to somehow revive this dead horse, into which health care reform has turned medical facilities, is rather strange. According to the logic of the Ministry of Health, the more money will be taken from a medical institution, the better it will work. Although, I think, this measure will not play a big role, on the contrary, it will kill the barely living remnants of healthcare, and somewhere it will die a little faster by its death.
Answering a question about the criteria by which the quality of medical care will be assessed if the initiative of the Ministry of Health adopts the force of law, Mr. Halperin noted that this is primarily about the requirements that the Ministry of Health issues again.
– In our country, unlike the entire civilized world, the Ministry of Health sets treatment standards and writes clinical recommendations, whereas in the West it is customary for medical professional communities to do this.
The expert added that the number of complaints as a result of the introduction of a new administrative punishment will only increase, and doctors will leave hospitals even more intensively. All this, the president of the Doctors Defense League emphasized, threatens that Russia will be left completely without healthcare professionals and what will happen again during the privatization of industry, when Russia was virtually left without engineers.
